fcgipp.h was in /usr/local/include
symlinking to /usr/include solved it.

I do not understand what you mean by 'symlinking' could you please
explain?

In my case the problem was:
# cc -E -I. -I/usr/lib/ruby/1.8/i486-linux -I. -fno-strict-aliasing -g
-g -O2 -fPIC conftest.c -o conftest.i

bash: cc: command not found

The solution is to install gcc
